I also had that problem and solved it the same way you did. 
It seemed to be related to Dataminer. Perhaps with the new Data Catalog this
problem will disappear.

I have that problem of added layers not showing up in the TOC in 9.1.  But
rather than toggle data frame to layout, I toggle the source and display tabs
at the bottom of the TOC.  Since no one else in the office has this problem
the only unique feature to my computer is that I'm running a XP pro 64 OS.
