[Pharmwaste] San Francisco Regional DEA Take Back Initiative Submittal Form

This is the updated version, changed from last week to allow, I think, more flexibility in collection event logistics, which was definitely needed.  Our DEA in MN is working with us on this same protocol and has asked for additional input.  We have additional notification requirements and HHW rules that come into play for these events.  There is a good chance MN will have several events (without a major incident) and that the collected pharms will be properly incinerated. There is much to do yet to prepare.
