or psychiatric nurse within the scope of his/her license must provide these
services. Services are provided in accordance with MaineCare definitions and rules
as specified in the MaineCare Benefits Manual. <o:p></o:p></span></font></p>

providing psychiatric medication services must have a position, generally
called a medical director, who is accountable for medical quality issues. This
position is a Maine-licensed physician with board-certified or board eligible
specialty training in psychiatry. This position assumes medical responsibility
for clinical staff performing medically necessary services under this service, including
staff selection, training and supervision. Additionally this position is
responsible for approving the organization’s system to identify
individuals in need of psychiatric evaluation and medication services.<o:p></o:p></span></font></p>
<p class=MsoNormal style='margin-left:.25in;text-autospace:none'><font size=3
face="Times New Roman"><span style='font-size:12.0pt'><o:p> </o:p></span></font></p>

annual medication review will be in all charts which include listing of all prescribers

into services a determination of a primary medical home shall be documented or
referral with the assistance of MaineCare and/or Schaller Anderson will be
documented. Release of information and coordination of care will likewise be
