Dear Small Quantity Generator County coordinators

Attached are the lists of facilities who have notified DEP and EPA that they
generate Hazardous Waste that either have a good ID match with the
information we have in CHAZ SQG, or there is no ID match in the County CHAZ
SQG data , and lastly if it was a bad ID match. This information is divided
into Districts and then again into counties.

The good ID match means that the listed CHAZ Notifier's address matched with
the information in CHAZ SQG. These generators should have the Listed EPA ID
numbers in their CHAZSQG data.
The no ID match means that there was no address match or EPA ID match. These
are sites who have sent DEP notifications of regulated waste activity
(8700-12) yet are not listed in CHAZSQG. These may well be long out of
business, but there are some that may be sites that should be added to the
assessment rolls.

The bad ID matches mean that the EPA ID matched but the address was
different. These are the most important sites. We would like you to look at
this list carefully and either correct the EPA ID numbers in CHAZ SQG, or try
to find
out if the sites are actually using these incorrect IDs to ship waste. These
are potential violations that could getthe sites in trouble and we think it
would be helpful for someone to get out there before DEP does and try to
educate these facilities.

The data is saved at:

ftp://ftp.dep.state.fl.us/pub/SQG_Program/Postcards/

grouped by DEP district and county.

The File: Postcards and SQGs - 2007 conference.pdf in the root directory
is a guide and explanation of where the data came from and how it is
organized.

We would really like you to look at the file named badmatch.xls for your
county and try to correct the SQG data or put the facilities high on your
list of facilities to verify.

Thanks you for your help. Please call Jack Griffith (850) 245-8748 or Glen
Perrigan (850) 245-8749 or your district DEP folks if you need any help or
further explanation.
