I was wondering more about the format of the questionaire, questions
asked, etc. I want to ask questions that encourage the client to give us
useful feedback, and I was hoping if someone else had such a thing, we
could use that as a template.

I use program called Perseus Survey Solutions. I lets you create your
own personal survey or used dozens of preloaded templates. The survey
can either be printed, emailed, or posted to a website.  All responses
are emailed to your inbox and the program automatically retrieves and
imports the results into a database. I think you can downloaded a trial
copy from their website.
