Hi, Glen –

We put the URLs for the 8700-12FL form, and for Publications, onto business-card-sized mailing labels.  We carry these conveniently within our ID or business card cases, and whenever we need to give this info to businesses it’s a simple thing to peel-and-stick the label to the back of a business card.  This saves paper, clutter, and ensures that the generator will always access the most current copy of the form or publications.

Hi All
When handing out the Notification of Regulated Waste Activity form 8700-12FL (to get an EPA ID number) during your verification site visits, please make sure you have the latest version. The latest version is dated 1/4/2009. Older forms submitted will be returned to the generator.

Here is a link to the 8700-12FL form and instructions. Please save this in your browser favorites since the form changes from time to time. I will send an email to you if it changes again.
